Empowering our Community with Path Care Social Foundation

Path Care Social Foundation provides inclusive, community-driven support services designed to uplift individuals and families across diverse needs and life stages. We empower women and girls, including survivors of abuse, support youth with skills development and employment readiness, and engage seniors through programs that reduce isolation and promote well-being. Our work extends to newcomers, immigrants, and refugees, as well as low-income individuals and families facing poverty, by connecting them with essential resources and opportunities. We foster social inclusion for those experiencing loneliness, offer mental health and emotional support, and guide job seekers through career training and mentorship. Additionally, we nurture emerging artists and creatives, assist caregivers with referrals and support, and inspire volunteers and community members to take on leadership roles and actively contribute to building stronger, more connected communities.

SERVICES

1. Safety, Healing and Well-Being

(Core for women, survivors, families, isolated individuals)


We provide compassionate, trauma-informed support services that prioritize emotional well-being and community care. Through trusted referrals, we connect individuals to counseling for mental health, family challenges, and domestic violence support. We facilitate women and girls' support circles that foster healing, confidence, and peer connection, while also offering seniors regular wellness check-ins and social clubs to reduce isolation. Our caregiver support groups and respite referrals ensure those caring for others receive the help they need, and our stress management, mindfulness, and emotional wellness workshops equip individuals with practical tools to build resilience and maintain overall well-being.

2. Life Skills, Independence, and Poverty Reduction

(For low-income individuals, newcomers, youth, and seniors)


We offer practical, accessible programs that equip individuals and families with essential everyday skills for greater independence and stability. Our financial literacy and budgeting workshops help participants manage their money with confidence, while our healthy living and nutrition sessions focus on affordable, culturally relevant choices. We provide training in daily life skills such as digital basics, form-filling, and navigating essential services. In addition, we support individuals in accessing housing, legal, and social services through guided referrals and offer newcomer orientation and non-legal settlement guidance to help individuals and families successfully adapt and thrive in their new communities.

3. Youth Employment Readiness

(For youth, job seekers, newcomers, women returning to work)


We offer comprehensive employment and skills development programs designed to help individuals build confidence and access meaningful opportunities. Our job readiness training covers resume writing, interview preparation, and essential workplace skills, while career exploration sessions and mentorship circles provide guidance and direction. We deliver digital skills and basic technology training to ensure participants are equipped for today’s workforce, and create volunteer-to-employment pathways that support hands-on experience and career transitions. Additionally, we introduce entrepreneurship basics for creatives and service workers, empowering individuals to develop their ideas and pursue sustainable self-employment.

4. Creative Empowerment and Arts for Change

(For emerging artists, filmmakers, creatives, youth, women)


We provide dynamic creative programs that nurture talent and amplify diverse voices through hands-on learning and community engagement. Our film and media production training offers entry-level, practical experience, while arts and storytelling workshops explore visual arts, writing, and performance. Participants benefit from mentorship by industry professionals, gaining valuable insights and guidance. We also create opportunities for community exhibitions, screenings, and showcases, and run artist development labs that specifically support and elevate marginalized voices, helping creatives grow, express themselves, and connect with wider audiences.

5. Active Living, Wellness and Social Connection

(For seniors, women, youth, isolated individuals)


We promote active, connected, and healthy lifestyles through inclusive recreation and wellness programs for all ages. Our offerings include gentle fitness, yoga, movement, and wellness classes designed to support physical and mental well-being, as well as intergenerational recreation programs that bring together people of different age groups. We organize community walks, outings, and cultural trips to encourage engagement and exploration, and host social cafés, hobby clubs, and peer networking opportunities to build meaningful connections. Our loneliness-reduction initiatives specifically support seniors and caregivers, helping them stay socially connected, supported, and engaged in the community.

6. Community Engagement, Leadership, and Volunteerism

(For volunteers, civic-minded community members)


We foster strong, engaged communities by creating meaningful opportunities for leadership, participation, and connection. Our volunteer training and leadership development programs equip individuals with the skills and confidence to make a positive impact, while youth leadership and civic engagement initiatives inspire the next generation to become active changemakers. We host community dialogues and cultural exchange events that promote understanding and inclusion, and facilitate intergenerational mentoring programs that connect youth and seniors for shared learning and support. Through community ambassador and peer supporter roles, we empower individuals to take on active responsibilities and contribute to building a more connected and resilient community.

7. Referral and Navigation Support (Cross-Cutting Service)

We provide personalized support services to help individuals navigate complex systems with confidence and dignity. Through one-on-one needs assessments, we identify each person’s unique situation and connect them with appropriate resources, including shelters, legal aid, healthcare, housing, and employment services. Our team offers ongoing follow-up support and advocacy to ensure individuals receive the assistance they need, while our multilingual and culturally sensitive approach ensures that everyone feels understood, respected, and supported throughout their journey.
